As a prospective student-athlete, some coaching staffs will ask for your participation in a tryout. If this is the case, then the form below, "prospective student-athlete tryout consent form", needs to be completed and returned either by fax or email. This needs to be completed at least 72 hours prior to the actual tryout date (i.e. if the tryout is on a Thursday, then the forms need to be to the department of sports medicine on Monday afternoon). Any individuals that do not have their tryout forms available 72 hours in advance will not be able to participate in the scheduled tryout. When being recruited, a full medical disclosure needs to be completed and returned to the University of Colorado at Colorado Springs Athletic Medicine Department. The link below is the form that must be completed and sent to the University of Colorado Colorado Springs Athletic Medicine Department before a letter of intent is sent for singing.
